Membership Terms

Effective Date: 8-01-2026

These apply to Essential, Enhanced, and Elite memberships unless otherwise stated.

  1. Membership Commitment: 

    1. All memberships require a minimum 3-month commitment.

    2. Memberships may be paused or canceled anytime after the initial 3 months with written notice (email, text, or in-clinic request).

  2. Billing & Payments:

    1. Membership fees are billed monthly on the date of enrollment.

    2. A valid card must remain on file for all active memberships.

    3. Failed payments must be resolved within 7 days to maintain active benefits.

  3. Use-It-or-Lose-It Policy:

    1. Your monthly IV and free shot must be used within the same billing cycle.

    2. Unused services do not roll over, accumulate, or convert to credit.

  4. Notification for Non-Use:

    1. Memberships with 3 consecutive unused months will be notified via email or phone. 

    2. If cancellation is needed, clients may rejoin at any time at the current membership pricing.

  5. Pausing Membership:

    1. After the 3-month minimum, memberships can be paused for:

    2. Travel

    3. Medical reasons

    4. Seasonal schedule changes

    5. Pause length: 1–3 months.

    6. Membership benefits cannot be used during a pause.

  6. Refund Policy:

    1. Membership fees are non-refundable once billed.

    2. Partial refunds for unused portions of the month are not provided.

    3. Memberships cannot be transferred or shared.

  7. Appointment Scheduling:

    1. Members receive priority scheduling, but availability is not guaranteed.

    2. Last-minute cancellations or no-shows follow standard clinic policy.

  8. Discounts & Perks:

    1. Members receive 10% off all IM injections and retail products after completing their monthly IV.

    2. Member Menu pricing is available only to active members.

    3. Discounts cannot be stacked with other promotions unless stated.

  9. Free Monthly Shot:

    1. Each membership includes one free shot per month based on tier-level price limits (see membership-specific terms below).

    2. Free shot cannot be substituted for cash, credit, or a higher-value service.

  10. Pricing Changes:

    1. Optimal IV Hydration & Wellness reserves the right to adjust membership pricing or benefits.

    2. Members will receive 30 days’ notice before any changes take effect.

  11. Medical Eligibility:

    1. All services require Medical Director/RN approval and/or medical clearance.

    2. If medically contraindicated at the time of visit, the membership benefit may be rescheduled, but not refunded.
